Wouldn't it be prudent to include the specific revision that was reviewed (ideally by using a permalink)? I can imagine that someone who'd put his name to a review would not want misinformation introduced later on? I think we can probably just add it in as a parameter for the existing template; the revision id is clearly visible from the permalink. Maybe the instructions to the reviewer should be to "use the permanent link" and include that URL in his review report.
