You asked:

Who gathered all of the books of the Bible together? No simple answer to that question. See Development of the Old Testament canon and Development of the New Testament canon.

Who did the editing? For some books, namely the books of the Pentateuch, no one can tell, as whoever did the final compilation is unnamed. But for others, including most of the New Testament, there is strong manuscript evidence that, aside from minor changes attributable to copist error more than anything else, the text we have is the same text that was written by the Apostles.

Who decided in what order that the Bible would be bound in? As far as I can tell, no one has done any research in that area. This is like asking why does A come before B in the alphabet.

What date was the Bible introduced as the Bible? Assigning a precise date to that is next to impossible. The Bible is a collection of all the writings that are regarded as canonical by Christians. Again, I must refer you to Development of the Old Testament canon and Development of the New Testament canon.
