In your section under "disability studies and medical humanities" I think there are a few edits you could make it make it more clear. First, I am unaware of what the social model of disability is. In reading this paragraph, I feel like you could maybe start with defining it. Then go on to explain its expansion into chronic illness etc. Also, you mention discrimination, maybe if it applies to the section expand on how that exists within medical humanities.
