There's a couple of issues that need to be addressed before it's restored:
  • Where did the image come from? If it's a personal scan, that's OK, just needs to be stated clearly
  • When was it issued? From the dates, it appears it was issued on 2/15/1979 (and expired on 3/24/79).
If I'm reading it correctly, the correct license would be {{PD-RU-exempt}}

Hello Skier Dude! Please, have a look at another Soviet Visa of the same type, but given to another person. Image link is below. Hope this time there will be no problems since for her it's OK if her sirname will be written. This visa is granting permission to leave the USSR forever for Кассель Елена Вадимовна (Kassel Elena Vadimovna, translation may vary) through Leningrad airport during the period from 24 Jan 1979 till 15 Feb 1979, who has personally scanned this Visa.
