I am not sure how I could "elaborate further" than a set of step-by-step instructions. You will need to select a new username, check that it is not already in use (step 3 tells you how to do that), and place the template on this page as instructed in step 1. Then once you're unblocked, go here to make the actual request to change your username. There really is no more "elaboration"; what it says there is what there is to it. You say "I really need to be unblocked". What is your need to be unblocked? Is it related to your conflict of interest?
