Well, if you can't get it to work, you've come to the right place. This is the documentation for my inquiry form.

Purpose

The sole purpose of this template is to ask for help on my talk page if you need help. In reality, this template is very simple to use. The trick is in knowing the fields and knowing how to use this template.

Fields

If you didn't know already, every template (well, almost) has fields. This template has two fields: a message field, and a username field. The following demonstrates the fields.

Just substitute "Subject", "Message field", and "Username field" with whatever you want. All fields are mandatory.

Subject

The subject field is for putting a chapter in the table of contents. It usually embodies the message or the question asked.

Message field

You can substitute the message field for whatever your question is that you have for me, as long as it's Wikipedia-related. For example, the following:

But, you can't have your inquiry form having "Sincerely, Username field". That's why I have a username field!

Username field

Just put your own username (without the prefix "User:") in this field, and it will indicate to me who messaged me, and how I can reach your userpage. The following:
