Use
[edit]This template creates a userbox indicating whether the user is alive or dead. Place this userbox template on your user page. To use this template, place the following line of Wikitext on your user page:

where:
date
is the check date or the date of death in yyyymmdd formatinfo-*
(Optional) are alternate messages to display for the three possible states
Please note: This template should not be used facetiously.
Elderly or ill users, or those suffering from hypochondriasis, may wish to let their fellow Wikipedians know that they are still kicking by periodically advancing the check date a few weeks into the future.
