[edit]

I remember buying this way back in the day, and what I bought was a single, rather large, hardcover book, rather than the boxed three-volume set described by Lawrence Schick or a set with GM screen, etc., as described here. I probably still have it, but if so it's packed in a storage unit in another state, which makes it kind of hard to check. This suggests that it was published in several formats. It's also notable that it said, if I recall correctly, that it was submitted in partial completion of degree requirements -- phraseology usually found in theses. In the history of RPGs this formed barely a blip, but the history of WAA itself might be worth a look; it's definitely not where the usual RPG (or thesis!) comes from.
