"Advanced Life Support (ALS) resources are sent to all calls - there may be a tiered response with a local fire department providing BLS first response via and engine or truck company, but the level of transporting care is always ALS and ALWAYS at the paramedic level."

I would like to point out that there are hybrid models,(in this example keep in mind ALL ALS transports are by Fire Rescue and ALL BLS transports are by private companies) ie. in Hillsborogh County, Florida calls into the EMS system are "triaged" meaning it could be dispatched ALS which would get an ALS(fire)Ambulance and/or ALS Engine for a call like chest pain. It could be dispatched ALS/BLS code 3 which would have an ALS Engine(fire) and BLS ambulanc (private)for a call like motor vehical collision or non tramatic abdominal pain and at that paient the ALS(fire medic) would asses the patient and make a level of care determination ALS vs. BLS and if the ALS(fire medic) determines the patient to be ALS at that time either cancell the on scene or still enrout BLS and upgrade to ALS Ambulance(fire)or if time constraints exsist bring his ALS gear onto the BLS unit and care for the patient with the BLS(private) EMT assisting, If the ALS(fire medic) determines the care to be BLS level then care would be handed over to the BLS(private) ambulance. The call could also be dispatched code 3 BLS(private) ie. ground level fall patient alert and breathing still on ground, then the BLS(private ambulace) EMT asses the patient and either transports or upgrades to ALS. Then there is BLS(private) code 2 ie. ear ache for 3 days.
