The author describes Preiser's Disease as Idiopathic. The definition of Idiopathic is the cause is unknown. With Preiser's Disease, the AVN (Avascular Necrosis) is caused by the compromised blood supply to the proximal pole of the scaphoid when it is fractured. The compromise is due to the unique blood supply to the scaphoid, which runs distal to proximal, which is unlike the other seven carpal bones. The blood is not being able to bridge the gap to the proximal fragment. Usually there is a secondary blood source from the ligaments attached to the distal radius, but occasionally this will not be adequate to maintain viability of the bone.
