An alternative viewpoint,and one that does not seem to have been considered at the time of the inquiry, is that it was not crew error which led to the vessel's loss but the fact that the chutes and their closures were of poor design, which effectively formed a 'design defect' in the watertight boundary of the hull.

I have often wondered why she sank in such a sea as she was designed for. As a professional geologist with more than a passing interest in ships and the oceans, I wonder if she sank for a reason that seems to be on everyone's lips these days but seemingly was never considered. The release of gas hydrate from the sea bed is known to occur, it causes the water to have the appearance of "boiling" and in fact the water is a massive area of escaping gases - similar to the effect of shaking a bottle of fizzy drink and releasing the top - what comes out is almost all gas and gas cannot support the displacement of a ship. The result is the ship falls (sinks) faster than the crew may be able to react and certainly making it impossible to send an SOS or MAYDAY call. I would like to see if that hypothesis holds up.
