exophthalmos vs. proptosis

I was taught that exophthalmos (as in Graves) features a lid lag when the patient looks downwards... ie, the sclera is visible above the iris... and that propotosis is just a buldging forward of the eye associated with hyperthyroidism in general and without a lid lag. ECD < histiocytosis

ECD is the only inflammatory cause listed, but other histiocytoses can also induce proptosis; see for example <http://www.nejm.org/doi/full/10.1056/NEJMicm1312532>.
