Here are some examples of citations which include original information as well as a link to The Cochrane Library. There's no mandatory form for citations. You should follow whatever citation style has been chosen for the relevant article, e.g., using citation templates or not. Wikipedia's rules about bare URLs and link rot require editors not to add only an unlabeled link to the Cochrane Library (or any other website). You should provide the normal citation information for the source (i.e., author, title, date, etc.). You may provide a convenience link to the Cochrane Library(and Cochrane would presumably appreciate it); if you do so, it should be labeled as "subscription required".

Tools

There are a number of simple tools that will automatically format references based on the PMID. Two include: diberri's tool and the toolserver. There is also a tool build into the WP:edit box known as Wikipedia:RefToolbar/2.0. .
