Photographer: p. xv of the work mysteriously attributes the photograph to "from the series made for this edition under the auspices of the Centre for Documentation and supplied to UNESCO in 1963." So an unknown worker on the Nag Hammadi project pre-1963, I can only presume.

Note: Some institutions attempt to claim copyright over photographs. However, US copyright law does not in general respect a new copyright as applying to an accurate reproduction of a work out of copyright. (See en:Bridgeman Art Library v. Corel Corp.) As the original manuscript is from the 4th century, that means any claim of copyright being retained is invalid.
